Care Coordination Form

A care coordination form is the first *short* communication a physician, family doctor or other healthcare practitioner will receive from our practice - Body Balance Nutrition. These letters are an important part of our care package for clients AND how we begin the process of gently educating our colleagues on the non-diet approach and weight inclusive care. Using a CCF is also a great way to network and market your practices unique point of difference and exceptional communication style with doctors and allied health professionals. It's a win-win for everyone!
